AHRQ’s Surveys on Patient Safety Culture™ (SOPS^®) Databases will open for voluntary data submission for organizations that have administered the following surveys:

• Hospital Survey on Patient Safety Culture 2.0 (Hospital Survey 2.0): June 1 – July 22

Hospitals can submit data from the:

–  SOPS Hospital Survey 2.0

–  Health IT Patient Safety Supplemental Items

–  Workplace Safety Supplemental Items (NEW)

–  Value and Efficiency Supplemental Items

• Nursing Home Survey on Patient Safety Culture: September 1 – 21

Participating facilities will receive feedback reports displaying their results as well as aggregated results from all database participants.
