Applications Open for the Rural Healthcare Provider Transition Project

Applications are being accepted for the Rural Healthcare Provider Transition Project (RHPTP) from the National Rural Health Resource Center. Selected rural hospitals and Rural Health Clinics will receive technical assistance (TA) in their efforts to strengthen their organization’s foundation in the key elements of value-based care (VBC) including but not limited to efficiency, quality, patient experience, and safety of care. The goal is to guide, prepare, and position rural hospitals and clinics to be an effective participant in a health system focused on value.

TA offered to the project’s five annual participants includes:

  • In-depth comprehensive consultations and coaching with subject matter experts to support the implementation of best practices and adoption of transition to value-based care strategies to prepare organizations for alternative payment models. Consultations may target clinical areas of efficiency, quality, patient experience, or safety of care.
  • An assessment of organizations’ readiness to assume financial risk.
  • Participation in a virtual peer-to-peer learning collaborative intended to support project implementation and sustainability.
  • A Health Education Learning Program (HELP) webinar series, with sessions focusing on advanced payment models and/or other related VBC topics.
  • Participation in post-project Recommendation Adoption Progress (RAP) calls which document progress, successes, and provide additional follow-up and support for project implementation.
