IMPACT Provider Survey on Patient Navigator Model

The IMPACT WV project is working with communities to create opportunities and address the needs of children diagnosed with Neonatal Abstinence Syndrome (NAS) or exposed to substances in utero.

The IMPACT WV Patient Navigator is responsible for coordinating the care of multigenerational families impacted by substance use and NAS. Services focus on meeting “where the client is,” by responding to current client needs such as food, shelter, education, clothing, transportation, and/or childcare. The three IMPACT Demonstration Sites, Wheeling Hospital Perinatal Transition Program, WV Court Appointed Special Advocates (CASA), and Burlington United Methodist Family Services (BUMFS), use a Patient Navigator to coordinate services.

This short survey will help us better understand the processes that facilitate or impede the
implementation of patient navigation models of care regarding mothers who are using or have used
substances. Responses will be used to inform training and technical assistance content regarding
patient navigation in rural settings.

Your participation in this project will take approximately 5-10 minutes to complete.

Your involvement in this project will be kept as confidential as legally possible. All data will be reported
in the aggregate. You will not be asked any questions that could lead back to your identity as a
participant. Your participation is completely voluntary. You may skip any question that you do not wish
to answer and you may discontinue at any time.
