NHSC Scholarship Application: Preparing Your Documents

 

If you plan to submit a National Health Service Corps (NHSC) Scholarship Program application, we encourage you to gather the following documentation, which is detailed on pages 41-44 of the Application and Program Guidance:

  • Proof of Status as a U.S. Citizen or U.S. National – submit a copy of a valid U.S. passport, passport card, U.S. birth certificate, or U.S Naturalization Certificate (Note: A driver’s license or a Social Security card are not acceptable.)
  • Authorization to Release Information Form – authorizes entities identified on the form to disclose information regarding those who have been selected and accept the scholarship award; must be dated and signed
  • Acceptance Report/Verification of Good Standing – documentation from the school verifying your acceptance or enrollment in good standing; completed and signed by the school official
  • Current Tuition and Fees Schedule – for the 2022-2023 school year or, if not yet available, the most recent tuition and fees schedule published by the school
  • Academic Letter of Recommendation – should be from a department chair, faculty advisor, or a faculty member of the academic program who can attest to your qualifications and include a handwritten or digital signature
  • Non-academic Letter of Recommendation – can be from an employer or previous employer, community leader, colleague, or anyone who has knowledge of your interest and motivation to provide care to underserved communities; the letter may include a personal address or organization if letterhead is unavailable and include a handwritten or digital signature
  • Responses to Three Essay Questions – detailed in the Application and Program Guidance, each response should be limited to a 500 word count or less
  • Transcript – from the last degree earned and the current degree program; an unofficial transcript is acceptable, but it must display the applicant name, school name, and grade point average (GPA)
  • Curriculum Vitae (CV) or Resume – outlines your relevant work/volunteer experience and should be no more than 5 pages long

Additional supporting documentation may also be needed (if applicable) and include the following: Verification of Disadvantaged Background and Statement Regarding Existing Service Obligation.
