Survey for Health Center COVID-19 Vaccine Program Participants

For Health Center COVID-19 Vaccine Program participants ONLY*:

HRSA is conducting a review of the Health Center COVID-19 Vaccine Program to support preparedness for future public health emergency response and programmatic planning. We would like to hear from program participants and supporting organizations throughout the health center network.

We have sent an email to the Project Director identified in HRSA’s Electronic Handbooks. This email came from [email protected] and has the subject line, “Your Experience with the Health Center COVID-19 Vaccine Program.”

The email contains a link to the survey. Your Project Director, or a single representative from your health center, should click the link to enter your responses. DO NOT reply to the email—use the link within it to share your input.

Share your answers by 11:59 p.m. your local time on Wednesday, July 20.

The short survey asks:

  • If and how the vaccine program helped your health center/organization increase equitable access to COVID-19 vaccines.
  • Which program aspects you found helpful.
  • Whether the vaccine program supported new or strengthened existing collaborations with public health and community partners.

We sent Project Directors for Primary Care Associations, Health Center Controlled Networks, and National Training and Technical Assistance Partners an email with a link to a similar survey. Those emails have the same subject line as mentioned above: “Your Experience with the Health Center COVID-19 Vaccine Program.”

Background

The Health Center COVID-19 Vaccine Program was developed to further ensure equitable access to COVID-19 vaccines in the nation’s underserved communities and those disproportionately affected by COVID-19. Since the launch of this program, health centers have held nearly 80,000 community-based vaccination events in partnership with local community-based organizations, including schools, community centers and faith-based organizations. Health centers have also used mobile vans to bring vaccines to convenient locations in the community. Nearly 70% of doses administered by health centers have been provided to individuals from racial and ethnic minority populations. HRSA recognizes and applauds health centers for their tremendous efforts during the public health emergency to protect their communities.
