Why are these data being collected? 
The Bureau for Public Health (Bureau) at the West Virginia Department of Health and Human Resources is currently developing the 2022-2026 HIV and Hepatitis C Elimination Plan as required by the Centers for Disease Control and Prevention (CDC) and the Health Resources and Services Administration (HRSA). The plan aims to accelerate progress toward meeting national goals while allowing West Virginia to design a service delivery system that reflects local vision, values, and needs.  

The results of this survey will be used to assist the Bureau’s Division of Sexually Transmitted Diseases (STD), HIV, Hepatitis, and Tuberculosis in better understanding the service needs and gaps within the state and informing the priorities of the state plan.  

The data submitted through this survey are confidential. Only a summary report will be published. No individual respondent information will be released. Thank you in advance for your participation. 
 
What will you be asked to do?   
The survey will take about 15 minutes to complete. There are two screening questions at the beginning of the survey, which determine if you are eligible to take the survey. 

The rest of the questions are optional. You can skip any questions you do not want to answer.
